Gojo let Geto go the first time because he couldn't hurt or kill his friend. Since then Geto had happily exploited and killed many people, people who would have lived if Gojo hadn't let him go.. I mean Akutami said that Geto's body voluntarily reacting to when Gojo called him is akin to when a dragonfly loses its head.
